Job summary

Guardian Angel Home Care Inc. is seeking a Quality Assurance Registered Nurse (QA RN) to join the Rochester Hills corporate team. This role focuses on reviewing clinical documentation, ensuring Medicare compliance, and supporting patient outcomes behind the scenes.

This fully in-office position collaborates with clinical leadership, field staff, and operations to maintain documentation accuracy, meet accreditation standards, and drive quality improvement initiatives.

Qualifications

• Active RN license
• Minimum 2 years Home Health experience
• Strong Medicare Conditions of Participation knowledge
• Experience reviewing OASIS documentation

Responsibilities

• Review and audit Home Health clinical documentation for accuracy and compliance
• Review and QA start of care (SOC), resumption of care (ROC), recertifications, discharges, evaluations and reassessments
• Review and approve physician orders and care plans
• Review and process 45-day summaries
• Ensure compliance with Medicare, State, and Accreditation standards
• Identify documentation deficiencies and communicate corrections to clinicians
• Work with HomeCare HomeBase (HCHB) system
• Assist with quality improvement initiatives
• Support branch offices and clinical teams with documentation compliance
