Job summary

Singh Management Co., Llc is seeking a Quality Assurance professional to oversee the QA program for Oakland Home Care. The role requires coordinating QA activities, analyzing data, and ensuring compliance with Medicare and regulatory standards.

The candidate should have a nursing or health care administration background with 3+ years in home care QA, knowledge of OASIS and QA processes, and experience with HCHB EMR systems. Part-time, on-site role in West Bloomfield, MI.

Qualifications

• Bachelor’s degree in nursing, health care administration, or equivalent program.
• Preferred QA licensing and coding experience.
• Preferred HCHB EMR experience.
• Minimum of 3 years clinical experience in a home care environment/QA.
• Demonstrated knowledge of Performance Improvement and Quality Assurance Programs.
• Knowledge of regulatory requirements for home care, including OASIS data collection and reporting requirements, as well as Home Care Value Based Purchasing, expectations for Medicare Certified Agencies.
• Ability to work under minimal supervision.
• Experience in research, analysis, and report preparation.
• Ability to solve complex problems using experience, knowledge, and reasoning.
• Work both as a part of a team and independently.

Responsibilities

• Coordinate with management team to ensure systems and procedures meet regulatory and quality standards.
• Review forms and reports, and implement changes or improvements as needed.
• Implement and monitor processes to evaluate safety, risk management, and infection control.
• Review records of active and closed patients to assure Medicare compliance and prepare summaries.
• Work with clinical staff to ensure documentation is complete and reflects delivery and legal requirements.
• Monitor utilization of services, client concerns, and risk management programs.
• Follow up on concerns from clients, families, employees, referral sources, and physicians.
• Maintain confidentiality in all aspects of the job.
• Promote personal safety and a safe environment for clients and staff.
• Perform duties in compliance with policies and professional/community standards.

Quality Assurance Job Description

• Implement and monitor processes to evaluate safety, risk management, infection control, and competency programs
• Review records of active and closed patients to assure compliance with Medicare requirements – prepare written summaries and recommendations
• Works closely with clinical staff to assure documentation is complete, accurate, and reflects delivery and legal requirements
• Monitors utilization of services, client concerns, and risk management programs
• Works with management staff to follow up on expressed concerns of clients, families, employees, referral sources, and physicians
• Maintains confidentiality in all aspects of the job
