Job summary

Senior Suites at Century Farms in Carrollton, OH is seeking an experienced Survey Readiness and Quality Assurance Nurse to support two assisted living communities. You will ensure ongoing compliance with Ohio Department of Health regulations, maintain survey readiness, and lead quality improvement initiatives across both locations.

Ideal candidates hold an active Ohio RN or LPN license, at least an associates degree, and 3 years of relevant LTCA/ALC experience.

Qualifications

• Active Ohio RN or LPN license in good standing.
• Minimum 3 years of experience in Assisted Living or Long-Term Care.
• Strong background in survey preparation and regulatory oversight.
• Experience developing and managing Plans of Correction.

Responsibilities

• Float between Century Farms Assisted Living and St. Clair Commons Assisted Living as needed.
• Maintain survey readiness per Ohio Department of Health regulations.
• Audit resident care plans, medication administration, documentation and infection control.
