Senior Suites at Century Farms is seeking an experienced Survey Readiness & QA Nurse to support two Assisted Living communities in Ohio. The role focuses on ensuring ongoing compliance with Ohio Department of Health regulations, maintaining survey readiness, and leading quality improvement initiatives across both locations.

The Nurse will float between Century Farms Assisted Living in Carrollton and St. Clair Commons in St.

Qualifications

• Active Ohio RN or LPN license in good standing.
• Minimum 3 years of experience in Assisted Living or Long-Term Care.
• Strong knowledge of survey preparation and regulatory standards.
• Experience developing Plans of Correction (POCs).

Responsibilities

• Maintain ongoing survey readiness across two communities.
• Lead audits of care plans, medication administration, and clinical documentation.
• Coordinate corrective actions and monitor compliance.
• Develop and oversee Plans of Correction for ODH citations.
• Provide staff education on regulations and documentation standards.
• Collaborate with Administrators and Leadership to address risk areas.
• Prepare facilities for state surveys and mock surveys.
