Quest Healthcare Development seeks a Survey Readiness & Quality Assurance Nurse to support Century Farms Assisted Living and St. Clair Commons Assisted Living in Ohio.

The role ensures ongoing compliance with Ohio Department of Health regulations and leads quality-improvement initiatives across both locations. The ideal candidate is organized, audits—driven, proficient in corrective action planning, and able to educate staff.

Qualifications

• Active Ohio RN or LPN license in good standing.
• Minimum 3 years of experience in Assisted Living, Long-Term Care, or similar setting.
• Experience with survey preparation and corrective action planning.
• Knowledge of Ohio Department of Health (ODH) regulations and Plans of Correction.

Responsibilities

• Float between Century Farms Assisted Living and St. Clair Commons Assisted Living based on survey priorities.
• Maintain ongoing survey readiness in accordance with ODH regulations.
• Conduct routine audits of resident care plans, medication administration, clinical documentation, infection control, and resident rights compliance.
• Lead and prepare facilities for state surveys, including mock surveys and readiness assessments.
• Develop and oversee Plans of Correction (POCs) for ODH citations.
• Monitor corrective actions to ensure sustained compliance.
• Provide staff education on ODH regulations, documentation standards, and infection prevention.
