Northgate Health Care Facility is seeking an experienced Pulmonary Program Coordinator (RN) to oversee and coordinate our Pulmonary Program. In this role, you'll provide clinical leadership, coordinate pulmonary-focused care, and collaborate with the interdisciplinary team to support residents with respiratory and pulmonary needs.

Qualifications

• Active New York State RN license required.
• Experience in skilled nursing, rehabilitation, pulmonary or subacute care preferred.
• Strong leadership and care coordination skills expected.

Responsabilités

• Oversee and coordinate daily operations of the Pulmonary Program.
• Coordinate individualized care plans for residents with pulmonary needs.
• Monitor residents with respiratory conditions and collaborate with the team.
• Assist with admissions, assessments, and care coordination for pulmonary residents.
• Provide staff education on pulmonary care and clinical best practices.
• Ensure compliance with regulatory and clinical standards.
• Participate in quality improvement and program development initiatives.
