Brookhaven Health Care Facility in East Patchogue, NY is seeking an experienced Pulmonary Program Coordinator (RN) to oversee and coordinate our Pulmonary Program. In this role, you'll provide clinical leadership, coordinate pulmonary-focused care, and collaborate with the interdisciplinary team to support residents with respiratory needs.

Active New York State RN license is required, with experience in skilled nursing, rehabilitation, or pulmonary care preferred.

• Assist with admissions, assessments, and care coordination for pulmonary residents.
• Provide staff education on pulmonary care and clinical best practices.
• Ensure compliance with regulatory and clinical standards.
• Participate in quality improvement and program development initiatives.

Equivalent Hourly Rate: $44/hour
Shift: Full-Time

• Strong leadership, organizational, and communication skills
