Qualifications

• Active New York State RN license required.
• Experience in skilled nursing, rehabilitation, pulmonary, respiratory, or subacute settings preferred.
• Strong clinical assessment and care coordination skills; ability to lead interdisciplinary teams.

Responsibilities

• Oversee daily operations of the Pulmonary Program.
• Coordinate individualized care plans for residents with pulmonary needs.
• Monitor residents with respiratory conditions and coordinate respiratory care.
• Collaborate with physicians, respiratory providers, therapy, nursing, and other teams.
• Assist with admissions, assessments, and care coordination for pulmonary residents.
• Provide staff education on pulmonary care and clinical best practices.
• Ensure compliance with regulatory and clinical standards.
• Participate in quality improvement and program development initiatives.

Equivalent Hourly Rate: $44/hour
