Job summary

Live Well Homecare is seeking a Registered Nurse for the Pulmonary Medicine Unit to care for higher acuity patients. You will develop and evaluate care plans, assess health data, and communicate with patients, families, and the care team to support disease management.

The role includes providing psychological support, assisting with injections, IV management, and oxygen therapy, while participating in quality improvement and professional development initiatives.

Qualifications

• Associate Degree or higher in Nursing; active Illinois RN license; ACLS and BLS certifications.
• Preferred: BSN and 1+ year inpatient experience.

Responsibilities

• Develops and evaluates care plans and assesses patient health data.
• Communicates with patients, families, and interdisciplinary teams; educates on disease management.
• Provides psychological support and assists with injections, IV management, and oxygen therapy.
• Participates in quality improvement, research, and professional development activities; wound/ostomy/skin care rounds if skilled.
• Ensures documentation and HIPAA compliance.
