Job summary

Responsibilities

• Demonstrates critical thinking to address patient care needs in ambulatory setting.
• Communicates clearly with care team, patients and families.
• Assists in onboarding and ongoing education of nursing staff.
• Supports care navigation and education on treatment plans and resources.
• Documents patient information accurately in EMR per policy.
