Job summary

Florida Department of Health in Miami is seeking a HUMAN SERVICES PROGRAM CONSULTANT II to coordinate and conduct interviews for pregnant patients with STDs, manage case records and coordinate with hospitals for treatment and referral.

The role requires field investigations, data tracking, and collaboration with DOH clinicians to ensure linkage to care for HIV and other services. English proficiency is essential; bilingual skills are preferred. Location is Health District Center, Miami, FL.

Qualifications

• Two years of experience in a disease control or related social services program.
• Preferred bilingual English/Spanish or English/Creole.

Responsibilities

• Coordinate and conduct timely interviews of pregnant patients with STD infections.
• Maintain and review field records and tracking logs weekly.
• Collaborate with hospitals, clinics and DOH staff for partner notification.
• Link clients to medical treatment and services as needed.
• Prepare documents for case review and ensure proper record abstraction.
