Staffing Solutions Organization LLC (SSO) is seeking a compassionate Public Health Social Worker in Augusta, Maine to support families through Maine CDC programs. The role blends in-office collaboration at 286 Water Street, Augusta, with statewide outreach and family navigation.

This position offers a Monday–Friday schedule, 8:00 AM–5:00 PM, a hybrid work arrangement, and meaningful case management responsibilities across two Maine public health programs.

Qualifications

• Current licensure in a healthcare or human services field such as Social Work, Nursing, Counseling or related behavioral health.
• Experience in case management, care coordination, or family support services.
• Knowledge of community services and support programs; ability to work independently with multidisciplinary teams.

Responsibilities

• Conduct family needs assessments and develop service connection plans.
• Coordinate referrals to medical, social service, housing, transportation, and support programs.
• Document family interactions, referrals, and case outcomes.
• Maintain collaborative relationships with healthcare providers and community partners.
• Support equitable access to healthcare and community services.
• Participate in home visits as needed.
• Assist families in overcoming barriers affecting child health and well-being.
