The role requires a Bachelor’s degree in Social Work or related field and a valid driver’s license. Experience in MNChoices/waiver case management and bilingual abilities are preferred.

Qualifications

• Bachelor’s degree in Social Work or related human services field or equivalent.
• A valid Driver’s license is required upon hire.
• One year of social work experience in home or community-based services preferred.
• Bilingual: English/Spanish and/or English/Somali preferred.

Responsibilities

• Provide case management, care coordination, and long-term care screenings to Rice County residents.
• Assess and evaluate clients' health and living situations to determine level of care and services needed.
• Coordinate client care with medical providers, vendors, clients, and families.
• Provide relocation assistance including assessment, community support plan development, and referrals as needed.
• Maintain accurate and up-to-date records.
• Other duties as assigned.
