Job summary

Urban Health Collaborative (TUHC) seeks a dependable, organized team member to support community programs, outreach, and program evaluation. You will work directly with community members to collect accurate information and help residents learn about TUHC services and local resources.

The role blends field-based outreach with data entry and administrative support, including surveys, participant intakes, outreach tracking, and follow-up activities under TUHC supervision.

Responsibilities

• Assist with collecting participant information through surveys and intake forms.
• Explain survey purpose and answer basic questions about data collection.
• Review completed forms for completeness and accuracy.
• Enter participant and outreach data into databases and tracking systems.
• Maintain organized records of outreach, participation, referrals, and follow-up.
• Prepare data for reports, grants, and internal evaluation.
• Follow confidentiality procedures for participant information.
• Conduct community outreach to raise awareness of TUHC programs and events.
• Engage community members at events and distribute program information.
• Support participant recruitment for programs, surveys, and workshops.
