Job summary

Mesa County Public Health is seeking a Public Health Nurse to provide nurse home visits and community health services across Mesa County. The nurse will assess clients’ physical, emotional, social, and environmental needs, connect families with resources, and document care within program guidelines.

Fluency in Spanish is desired, with CPR and Colorado RN licensure required. The role works in a team environment to support families, promote maternal and child health, and align with public health

Qualifications

• Bachelor’s Degree in Nursing (BSN) is required.
• Spanish language proficiency is preferred.
• Certified Lactation Educator or Consultant is preferred.

Responsibilities

• Perform home visits to assess physical, emotional, social, and environmental needs.
• Develop family plans and coordinate referrals to community resources.
• Maintain nursing documentation accurately and promptly.
• Collaborate with a team to support client care and program objectives.
• Promote public health programs and participate in ongoing professional development.
