Job summary

Coos Health & Wellness in Coos Bay, Oregon seeks a self-motivated nurse to join our home visiting team serving pregnant and postpartum individuals, infants, children, and youth with special health needs in Coos County. Fluency in Spanish is required.

The role includes completing nursing assessments, providing prenatal to parenting education, acting as a case manager, and documenting encounters. Sign-on bonus of $3,000 and relocation assistance may be available.

Qualifications

Must hold a current Oregon RN license.
Valid Oregon driver’s license.
Fluency in English and Spanish (read/write/speak).
Must successfully pass a criminal background check.
Prefer 2+ years of home visiting, early childhood development, psychology or related fields.

Responsibilities

Provide nurse home visits to pregnant people and children with special health needs.
Deliver prenatal, postpartum, pediatric and parenting education and support.
Perform comprehensive nursing assessments and periodic reassessments, and monitor care plans.
Serve as a case manager to develop, implement and monitor care, and link clients to resources.
Document encounters in the EMR and data collection systems.
