Job summary

Florida Department of Health in Miami-Dade County seeks an OPS Health Services Representative to perform on-site field audits of immunization-related records and verify compliance with state health standards. Responsibilities

• Conduct field audits by reviewing DH-680 forms in Miami-Dade County schools, private schools and daycares to verify compliance.
• Schedule audit visits and communicate with school and daycare administrators to track records in FL SHOTS.
• Educate facility staff on compliance and deliver site-level system training for FL SHOTS access.
• Notify VFC Team for any discrepancy in DH-680 forms and use company devices for audit-related actions.
• Produce follow-up results and upload audit documentation to TEAMS.
• Assist in immunization services during clinic operations when needed.
• Create and issue DH-680 and DH-681 forms in accordance with guidelines.
• Help ensure data entry accuracy in FL SHOTS and HMS systems.
