Job summary

Florida Department of Health seeks an OPS Human Services Program Consultant II to conduct disease intervention work in a clinical setting, including interviewing STD patients and partners for detection and prevention efforts. The role involves counseling on Pre-Exposure Prophylaxis (PreP) and linking patients to care in the community.

Responsibilities include case management, data reporting, and collaboration with HIV testing and care staff, as well as coordination with community organizations

Qualifications

• Experience in case management, medical record review, or public health investigations.
• Ability to work with clinical staff and external partners.
• Bilingual in English and Spanish or English/Creole preferred.

Responsibilities

• Perform embedded Disease Intervention Specialist work in a clinical setting to investigate disease prevalence.
• Conduct interviews with STD patients and partners for contact tracing and linkage to care.
• Educate patients about infections and connect them to PreP services and care providers.
• Coordinate with HIV testing linkage to care and community staff; report data as required.
