Job summary

Florida Department of Health seeks an OPS Human Services Program Consultant II to conduct disease intervention interviews, cluster investigations, and partner notification in a clinical setting. You will educate patients on infections, develop referral plans, and coordinate with HIV care providers within the community.

The role emphasizes data collection, reporting, and collaboration with DIS and clinical staff to improve prevention and intervention strategies.

Qualifications

• Minimum experience in case management or public health investigations.
• Ability to work collaboratively with clinical staff, Disease Intervention Specialists (DIS), and external partners.
• Bilingual English/Spanish or English/Creole preferred.
• Experience with data collection and reporting in public health surveillance.

Responsibilities

• Conduct interviews and re-interviews of STD patients to educate, refer, and educate partners.
• Coordinate with HIV testing linkage to care and providers in the community.
• Perform data entry and maintain accurate case records and dashboards.
