Palms at Venice in Venice, Florida is seeking a licensed Physical Therapist Assistant (PTA) to join our rehabilitation team on a PRN basis. The role focuses on helping residents regain mobility and independence in a supportive, resident-centered environment.

You will evaluate residents, create treatment plans, and provide interventions while coordinating with nurses and other therapists; documentation and discharge planning are key parts of the job.

Qualifications

• Current Florida Physical Therapist Assistant license is required.
• Experience in Skilled Nursing or Post-Acute Care preferred.
• Strong clinical judgment and communication skills.
• Ability to work independently and as part of a team.
• Commitment to high-quality resident care and outcomes.

Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans.
• Provide skilled therapeutic interventions to improve mobility, balance, strength, and endurance.
• Educate residents and families on safety, mobility, and adaptive techniques.
• Collaborate with nursing, therapy, and interdisciplinary team members.
• Maintain accurate, timely, and compliant clinical documentation.
• Participate in care plan meetings and discharge planning.
