Job summary

THREEMEADOWS is seeking a Staff Physical Therapist Assistant to support the Staff Physical Therapist in resident care and treatment planning. You will help implement treatment plans, monitor progress, and contribute to a collaborative care environment.

In this role, you will document accurately, maintain safety and infection control, educate residents and families on goals and home programs, and work with the interdisciplinary team to support discharge planning and quality care outcomes.

Qualifications

• Associate degree from an accredited PTA program.
• Proficient in computer skills with PTA software such as Net Health and Point Click Care.
• Licensed as a Physical Therapist Assistant in the state; maintain licensure and CE requirements.

Responsibilities

• Treat residents under supervision of Physical Therapist.
• Maintain accurate, timely documentation per regulations.
• Ensure safe treatment environment and infection control.
• Assist with training Restorative Aides when needed.
• Coordinate with care team on progress and discharge planning.
• Educate residents and families on treatment goals and home programs.
