Relient Health is seeking a full-time Physical Therapist Assistant (PTA) to join a Skilled Nursing Facility in Fremont, CA. You will provide skilled therapy under the supervision of a licensed PT, develop and implement treatment plans, and document patient progress.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.

• Provide skilled therapy services under the direction of a licensed Physical Therapist.
• Assist in developing and implementing individualized treatment plans.
• Document patient progress using EMR systems.
• Foster strong communication with PTs, OTs, SLPs, and nursing staff.
