Job summary

Blue Stone Therapy is seeking a Physical Therapist Assistant (PTA) to provide rehabilitation services to adult and geriatric patients at Mapleton Community Home in Mapleton, MN. You will work under a licensed Physical Therapist to support mobility and functional goals in a SNF setting.

Responsibilities include delivering skilled PT services, documenting progress, and collaborating with an interdisciplinary team to achieve meaningful outcomes.

Qualifications

• Graduate of an accredited Physical Therapy Assistant program.
• Current Physical Therapy Assistant license or eligibility in applicable state.
• Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred.

Responsibilities

• Provide skilled physical therapy treatment in accordance with established plans of care.
• Implement individualized treatment programs focused on functional and measurable outcomes.
• Treat adult and geriatric patients in a SNF setting and rehabilitation settings.
• Monitor patient response to treatment and communicate progress with the supervising Physical Therapist.
• Maintain timely, accurate documentation to support quality care and reimbursement standards.
• Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes.
