Job summary

Blue Stone Therapy in Inver Grove Heights, MN is seeking a Physical Therapist Assistant (PTA) to provide rehabilitation services to adult and geriatric patients in a skilled nursing/post-acute setting.

You will work under a licensed Physical Therapist to implement treatment plans that improve mobility, strength, balance, endurance, and overall functional independence, while delivering high-quality care and ensuring safety and regulatory compliance.

Qualifications

• Graduate of an accredited Physical Therapy Assistant program.
• Current Physical Therapy Assistant license (or eligibility).
• Experience in skilled nursing, long-term care, or post-acute rehab preferred.
• Strong communication, organization, and clinical reasoning with patient-centered care.

Responsibilities

• Provide skilled physical therapy treatment per plans of care.
• Implement individualized programs focused on functional outcomes.
• Treat adults and geriatric patients in SNF and rehab settings.
• Monitor response to treatment and communicate progress with the PT.
• Maintain timely documentation to support care and reimbursement.
• Collaborate with interdisciplinary team to improve patient outcomes.
