Blue Stone Therapy is seeking a Physical Therapist Assistant to support rehabilitation services at Woodlyn Heights Care Center in Inver Grove Heights, MN. You will work under a licensed PT to implement targeted treatment plans for adult and geriatric patients in SNF and post-acute settings.

Qualifications

• Graduate of an accredited Physical Therapy Assistant program.
• Current Physical Therapy Assistant license (or eligibility for licensure) in applicable state.
• Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred.
• Strong communication, organization, and clinical reasoning skills.
• Commitment to patient-centered care and professional standards.

Responsibilities

• Provide skilled physical therapy treatment in accordance with established plans of care
• Implement individualized treatment programs focused on functional and measurable outcomes
• Treat adult and geriatric patients in a skilled nursing facility (SNF) setting and rehabilitation settings
• Monitor patient response to treatment and communicate progress with the supervising Physical Therapist
• Maintain timely, accurate documentation to support quality care and reimbursement standards
• Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes
