Job summary

Blue Stone Therapy is seeking a Physical Therapist Assistant (PTA) to provide rehabilitation services to adult and geriatric patients at Mapleton Community Home. This role works under the supervision of a licensed Physical Therapist to implement treatment plans designed to improve mobility, strength, balance, and independence.

In addition to a $5,000 sign-on bonus for eligible full-time hires, the PTA will join a team dedicated to high clinical standards and collaborative care within a skilled

Qualifications

• Graduate of an accredited Physical Therapy Assistant program.
• Current Physical Therapy Assistant license (or eligibility for licensure) in applicable state.
• Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred.
• Strong communication, organization, and clinical reasoning skills.

Responsibilities

• Provide skilled physical therapy treatment in accordance with established plans of care.
• Implement individualized treatment programs focused on functional and measurable outcomes.
• Treat adult and geriatric patients in a skilled nursing facility (SNF) setting and rehabilitation settings.
• Monitor patient response to treatment and communicate progress with the supervising Physical Therapist.
• Maintain timely, accurate documentation to support quality care and reimbursement standards.
• Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes.
