Job summary

Mental Health Association of Rochester is seeking a Physical Therapist Assistant to provide patient treatment, exercise instruction, and education in a home care setting, helping clients improve mobility and daily functioning.

You will follow PT plans, assist with exercises and transfers, monitor progress, and coordinate with the healthcare team to ensure safe, high-quality care in diverse home environments. Weekend rotations may be required.

Qualifications

• Current licensure, registration, or certification to practice physical therapy.
• Valid driver’s license, vehicle insurance, or access to transportation.
• Availability for weekend rotations.
• Preferred: CPR certification and physical ability to assist with mobility and transfers.
