Responsibilities

• Assist physical therapists in developing, implementing, and evaluating individualized treatment plans for residents.
• Provide physical therapy interventions under supervision of a licensed physical therapist.
• Collaborate with the physical therapist to design and implement treatment plans addressing residents' physical, emotional, and social needs.
• Communicate effectively with residents, family members, and healthcare team members to ensure seamless care transitions.
• Participate in team meetings and contribute to quality improvement initiatives.
• Maintain accurate and detailed records of treatments, progress, and outcomes.
