Job summary

Fort Wayne Health Campus in Indiana is seeking a licensed Physical Therapist Assistant to support residents’ recovery and independence under a licensed Physical Therapist. You will deliver prescribed therapies, document progress, and help manage caseloads within OBRA guidelines.

The role emphasizes collaboration with the Director of Rehabilitation, infection control, and participation in routine in-services to ensure quality care.

Qualifications

• Active Indiana PTA license.
• Graduate of an accredited PTA program.
• Comfortable working under the direction of a licensed Physical Therapist.

Responsibilities

• Provide therapy to residents with musculoskeletal, neurological, balance, and gait needs.
• Deliver treatment per the physician order and the supervising PT’s plan of care.
• Document treatments, progress notes, and therapy records.
• Manage resident treatment schedules and caseloads.
• Flag caseload needs and resident screenings to the Director of Rehabilitation.
• Follow infection control policies and OBRA Resident Rights guidelines.
• Take part in facility meetings and required in-services.
