West Shore Post-Acute in Alameda, CA is seeking a Physical Therapist Assistant to assist Physical Therapists in delivering high-quality rehabilitative care to residents. The role focuses on mobility, strength, and functional outcomes under licensed PT supervision, welcoming both experienced therapists and new graduates.

Qualifications

• Active California Physical Therapist Assistant license in good standing.
• Associate degree from an accredited Physical Therapist Assistant program.
• New graduates are welcome to apply; experience in skilled nursing or rehabilitation settings preferred.
• Knowledge of PDPM and basic documentation standards.
• Strong communication, teamwork, and interpersonal skills.
• Compassionate, resident-centered approach to care.
• Physical ability to safely assist residents with mobility and therapeutic activities.

Responsibilities

• Implement physical therapy treatment plans under PT supervision.
• Document therapy sessions and resident progress accurately and timely.
• Assist residents with therapeutic exercises, mobility, and functional activities.
• Collaborate with Physical Therapists and the interdisciplinary team.
• Participate in care conferences and therapy-related activities, as applicable.
