Job summary

Hope Creek Nursing and Rehabilitation Center in East Moline, Illinois seeks a dedicated Physical Therapist Assistant to support the rehabilitation team in improving patients' mobility and function.

Under the supervision of a licensed Physical Therapist, you will design and implement individualized treatment plans, conduct sessions, monitor progress, educate patients and families, and maintain accurate records to ensure high-quality care.

Qualifications

• High school diploma or equivalent (required).
• Graduate of an accredited Physical Therapist Assistant program.
• Current PTA certification and licensure as applicable.
• Ability to work in a fast-paced environment with strong organization.
• Excellent communication and interpersonal skills.

Responsibilities

• Assist Physical Therapists in developing and implementing patient treatment plans.
• Conduct treatment sessions with exercises, modalities, and other interventions.
• Monitor patient progress and record treatment outcomes.
• Communicate with patients, families, and healthcare professionals to ensure care coordination.
• Participate in patient education and training programs.
• Maintain accurate and detailed records of patient care and treatment.
