Job summary

Hope Creek Nursing and Rehabilitation Center in East Moline, Illinois, is seeking a compassionate Physical Therapist (PT) to join our team. The PT will assess residents, design individualized rehabilitation plans, and implement evidence-based therapies to improve mobility and independence.

You will work with an interdisciplinary team to ensure seamless care transitions and ongoing progress toward optimal outcomes for residents in a fast-paced setting.

Qualifications

• Must be a licensed Physical Therapist (PT) with a degree from an accredited Physical Therapy program
• Must have a strong foundation in physical therapy principles, techniques, and best practices
• Must be able to work effectively in a fast-paced, dynamic environment with a focus on patient-centered care
• Must be able to lift, push, and pull equipment and supplies as needed

Responsibilities

• Conduct thorough initial and ongoing assessments to identify patient needs and develop individualized rehabilitation plans
• Implement evidence-based physical therapy interventions, including exercise programs, manual therapy, and modalities
• Collaborate with our healthcare team to develop and update patient treatment plans, ensuring seamless transitions and optimal outcomes
• Conduct patient education and training on proper body mechanics, safety, and self-care techniques
• Maintain accurate and detailed records of patient progress and treatment plans
• Participate in quality improvement initiatives to drive continuous improvement and excellence in patient care
