Job summary

ConnectedHealth is seeking a Physical Therapist Assistant to join a multidisciplinary rehabilitation team in Utah. You will deliver therapy under a licensed PT, support patient mobility, and help patients achieve independence through individualized plans.

You will document care, communicate progress to the team, and educate patients and families. Collaboration with physicians and nurses is essential to provide high-quality care and comply with regulations.

Qualifications

• Must have an Associate Degree from an accredited PTA program.
• Current Utah PTA license or eligibility to obtain licensure.
• Current CPR/BLS certification.
• Strong communication and interpersonal skills for collaborative care.
• Ability to document accurately and maintain treatment records.

Responsibilities

• Deliver physical therapy treatments under the direction of a licensed Physical Therapist.
• Assist patients to improve strength, mobility, balance, and independence.
• Implement individualized treatment plans designed by the supervising PT.
• Monitor patient progress and communicate outcomes to the therapy team.
• Accurately document patient care, progress notes, and treatment activities.
• Educate patients and families about treatment plans, exercises, and injury prevention.
• Collaborate with physicians, therapists, nurses, and other healthcare professionals.
• Maintain compliance with facility policies, state regulations, and standards.
