Job summary

ConnectedHealth is seeking a Physical Therapist Assistant to join a multidisciplinary rehabilitation team in Utah. You will deliver therapy under a licensed PT, support patient mobility, and help patients achieve independence through individualized plans.

You will document care, communicate progress to the team, and educate patients and families. Collaboration with physicians and nurses is essential to provide high-quality care and comply with regulations.

Qualifications

• Must have an Associate Degree from an accredited PTA program.
• Current Utah PTA license or eligibility to obtain licensure.
• Current CPR/BLS certification.
• Strong communication and interpersonal skills for collaborative care.
• Ability to document accurately and maintain treatment records.

Responsibilities

Job description

Physical Therapist Assistant works with a rehabilitation team to deliver physical therapy treatments, improve patient mobility, and support recovery. The role supports patients in achieving independence and functional goals under the guidance of a licensed Physical Therapist.

Key Responsibilities:

• Deliver physical therapy treatments under the direction of a licensed Physical Therapist
• Assist patients in improving strength, mobility, balance, coordination, and functional independence
• Implement individualized treatment plans designed by the supervising Physical Therapist
• Monitor patient progress and communicate outcomes to the therapy team
• Accurately document patient care, progress notes, and treatment activities
• Educate patients and family members regarding treatment plans, exercises, and injury prevention techniques
• Collaborate with physicians, therapists, nurses, and other healthcare professionals to ensure quality patient care
• Maintain compliance with facility policies, state regulations, and professional standards

Qualifications:

• Associate Degree from an accredited Physical Therapist Assistant program
• Current Utah Physical Therapist Assistant license or eligibility to obtain licensure
• Current CPR/BLS certification
• Strong communication, interpersonal, and organizational skills
• Ability to work effectively in a collaborative healthcare environment
