Job summary

Heritage Grove Estates in Topeka, KS is hiring a Physical Therapist Assistant (FT OR PRN) to join our in-house rehab team. New grads are encouraged to apply and you will provide therapy under a licensed PT.

Qualifications

• Active Kansas PTA license in good standing.
• Experience in skilled nursing or long-term care preferred.
• Experience with Net Health documentation is a plus.
• Strong communication and teamwork skills.

Responsibilities

• Provide prescribed physical therapy treatments to improve mobility, function, and pain management.
• Deliver skilled therapy services under the supervision of a licensed Physical Therapist.
• Support implementation and adjustment of individualized care plans based on resident progress.
• Track and document patients' strength, mobility, and response to treatment.
• Educate residents, families, and caregivers on therapy goals and techniques.
• Maintain accurate, timely documentation in compliance with company policies and state regulations.
• Communicate updates and clinical observations to the supervising PT.
• Participate in team meetings and collaborate with interdisciplinary staff.
