Job summary

Heritage Grove Estates in Topeka, KS is seeking a Physical Therapist Assistant to join the in-house rehab team. We offer a Full-Time or PRN schedule, a $3,000 sign-on bonus, and a patient-centered, team-based environment that welcomes new grads.

Qualifications

• Active Kansas PTA license in good standing.
• Experience in skilled nursing or long-term care preferred.
• Experience with Net Health documentation system is a plus.

Responsibilities

• Provide prescribed physical therapy treatments to improve mobility, function, and pain management
• Deliver skilled therapy services under supervision of a licensed Physical Therapist
• Support the implementation and adjustment of individualized care plans based on resident progress
• Track and document patients’ strength, mobility, and response to treatment
• Educate residents, families, and caregivers on therapy goals and techniques
• Maintain accurate, timely documentation in compliance with company policies and state regulations
• Communicate updates and clinical observations to the supervising PT
• Participate in team meetings and collaborate with interdisciplinary staff
