Continuum Therapy Partners is seeking a Physical Therapist Assistant to support patient care at Mt. Vernon Countryside Manor in Mt. Vernon, IL. You will work under a supervising PT to implement treatment plans and monitor patient progress.

The role emphasizes hands-on patient education, exercise programs, and device usage guidance, with a PRN, location-based schedule at our Mount Vernon facility.

Qualifications

• Requires an Associate Degree in Physical Therapist Assistant
• Current, unencumbered state PTA license
• One year of PTA experience preferred

Responsibilities

• Implements individualized treatment plans under the supervision of a Physical Therapist
• Documents patient progress and reports changes in patient status to the supervising PT
• Educates patients on home exercise programs and proper use of assistive devices

Physical Therapist Assistant Requirements & Responsibilities:

• Associate Degree in Physical Therapist Assistant Program required

• Current, unencumbered state Physical Therapist Assistant License

• 1 year of previous experience as a Physical Therapist Assistant preferred

• Implements individualized treatment plans under the supervision of a Physical Therapist

• Documents patient progress and reports changes in patient status to the supervising PT
