Job summary

The Legacy Senior Communities is seeking a licensed Physical Therapy Assistant (PTA) to assist with screening, planning, implementing and documenting therapy programs under the direction of a Physical Therapist. You will follow community guidelines to maintain standards of treatment in line with legal and accrediting requirements.

Responsibilities include reviewing screening results, planning therapeutic activities, communicating plans to patients and families, collaborating with the health care

Qualifications

• Must be a graduate of an accredited PTA program.
• Must hold a valid Texas license to practice as a Physical Therapy Assistant.
• Must have current Cardio Pulmonary Recitation Card (CPR) from AHA or Red Cross with hands-on practice or obtain CPR training within 30 days of hire date.

Responsibilities

• Review member, resident or patient status after PT screening and determine intervention needs.
• Plan therapeutic activities to meet patient goals.
• Communicate treatment plan and goals to patient and family.
• Collaborate with health care team for discharge or transfer planning.
• Implement interventions as per plan of care and document daily progress.
• Reassess and update plan of care as patient outcomes evolve.
• Notify PT of significant changes in patient status or intervention needs.
