Job summary

Innovate Rehab & Wellness in Kansas City, MO invites a Licensed Physical Therapy Assistant to join our Skilled Nursing Facility team. The PTA will work under the direction of a licensed Physical Therapist to help patients improve mobility, regain strength, and increase function through individualized, hands-on care.

We offer a full-time schedule Monday–Friday with competitive hourly pay based on experience and a supportive, collaborative work environment focused on clinical outcomes and job

Qualifications

• Associate degree from an accredited PTA program.
• Valid state PTA license.
• Rehab/clinical experience preferred (new grads welcome).
• CPR certification preferred.

Responsibilities

• Implement patient-specific treatment plans created by the PT.
• Lead patients through exercises, mobility training, and rehab activities.
• Educate patients on safe techniques and assistive devices.
• Monitor and document patient progress; report changes to the PT.
• Maintain accurate, compliant therapy records.
• Collaborate with the care team for coordinated treatment.
