Responsibilities

• Delivers patient care according to standards, physician orders, and care plan.
• Assists in patient assessment and tracks progress within scope of practice.
• Collaborates with evaluating therapists to support treatment goals and outcomes.
• Provides education to patients, families, and caregivers according to the plan of care.
• Supports discharge planning and re-assessment with therapist.
• Communicates effectively with care team to optimize patient outcomes.
• Applies clinical techniques, modalities, and equipment.
• Travels to local clinics as needed.
