Qualifications

• Current or eligible Physical Therapist Assistant license in the state of practice.

Responsibilities

• Treat residents with disorders or deficits within Physical Therapy scope after physician order and evaluation.
• Optimize residents’ functionality and skills.
• Complete documentation via Net Health.
• Collaborate with the supervising PT to develop programs and fulfill co-signature requirements.
• Attend care planning, staff, and family meetings as required.
• Assist in meeting OBRA standards and facility certification requirements.
• Follow infection control policies.
• Perform other tasks as assigned by manager.
