Job summary

Evolve Therapy Services is hiring a Physical Therapist for an in-house Skilled Nursing Facility in Humboldt, TN. This PRN position focuses on delivering high-quality physical therapy services in a facility setting and ensuring compliance with CMS expectations and company procedures.

You will assess residents, develop individualized treatment plans, document thoroughly, and collaborate with an interdisciplinary team to optimize resident outcomes.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards and procedures.
• Adhere to documentation and billing procedures for therapy services.
• Conduct screenings and evaluations to determine intervention needs and plan treatment.
• Develop and implement treatment plans with short- and long-term goals.
• Communicate findings and recommendations to residents, families, and the care team.
• Complete compliant documentation in NetHealth EMR systems.
