Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention.
• Assist in developing target lists with short-term and long-term goals and treatment plans.
• Conduct individual and group treatments as outlined in the treatment plan.
• Monitor residents' response to interventions and adjust as directed by the Physical Therapist.
• Establish and modify resident schedules in coordination with the Physical Therapist and team.
• Train residents, families and staff in equipment use and therapy techniques.
• Maintain compliant and medically necessary documentation in CMS NetHealth EMR.

Education

Associate's degree in physical therapy

Active Physical Therapy Assistant license in state(s) of practice

Continuing education to maintain licensure
