Job summary

Evolve Therapy Services, LLC is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility located in Hadley, MA. This full-time role offers a $2,500 sign-on bonus and PRN options.

The PTA will work under the supervision of a Physical Therapist to provide patient care, conduct screenings, and assist in treatment planning. License must be active in state(s) of practice, and ongoing continuing education is required.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Commitment to ongoing continuing education and licensure maintenance.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention.
• Assist in developing and implementing treatment plans and goals.
• Maintain compliant documentation in NetHealth EMR per CMS expectations.
• Participate in resident, family, and staff education.
