Job summary

Evolve Therapy Services, LLC is hiring a Physical Therapist for an in-house Skilled Nursing Facility in Hadley, MA. Full Time, Part Time, or PRN positions available with a $3,000 sign-on bonus.

Responsibilities include providing physical therapy services, evaluating residents, developing treatment plans, documenting progress, and ensuring CMS-compliant records. Active licensure and ongoing education are required.

Qualifications

• Bachelor degree in Physical Therapy from an accredited program.
• Current PT licensure in practice state(s).
• Commitment to ongoing professional development and licensure.

Responsibilities

• Provide PT services to residents per standards and productivity targets.
• Document and bill PT services per procedures.
• Evaluate and develop short/long-term treatment goals.
• Screen residents to determine need for intervention.
• Complete CMS-compliant documentation in NetHealth EMR.
