Carecore at Willowood is seeking a compassionate PT Physical Therapist Assistant (PTA) to join our in-house rehabilitation team in Brunswick, Ohio. Working under the supervision of a Physical Therapist, you will help residents improve mobility, strength, function, and independence with personalized, resident-centered care.

Qualifications

• Active PTA license in good standing.
• Skilled nursing or long-term care experience preferred.
• Strong clinical, communication, and organizational skills.
• Knowledge of the Net Health Electronic Documentation System preferred.

Responsibilities

• Provide skilled physical therapy treatments under the supervision of a Physical Therapist.
• Implement individualized plans of care to reduce pain and improve function.
• Monitor and document resident progress, including strength, mobility, and motor performance.
• Communicate changes in resident condition to the Physical Therapist and care team.
• Educate residents, families, and caregivers.
• Collaborate with the interdisciplinary care team.
• Maintain accurate, timely, and compliant documentation.
• Follow all professional, legal, and regulatory standards.
