Job summary

Gardens of Euclid Beach in Cleveland, Ohio is seeking a licensed Physical Therapist Assistant (PTA) to join our in-house rehabilitation team. This role supports skilled therapy under a licensed PT, with both Full-Time and PRN hours available and benefits for eligible candidates.

You will help residents improve mobility, balance, and independence through individualized plans, collaborate with nursing and interdisciplinary team, and maintain accurate documentation. Ohio PTA license required.

Qualifications

• Active Ohio PTA license.
• LTC/SNF or geriatric experience preferred.
• Strong teamwork and communication skills.
• Ability to work under supervision of PT.

Responsibilities

• Provide skilled therapy interventions under PT supervision.
• Implement individualized treatment plans.
• Document progress and outcomes accurately.
• Collaborate with nursing and rehab team.
• Educate residents and families on exercises.
