CareCore at Willowood in Brunswick, OH is seeking a compassionate PRN Physical Therapist (PT) to join our in-house rehabilitation team in a skilled nursing and long-term care setting. Flexible weekday and weekend opportunities, with an emphasis on collaborative care and documentation excellence.

The role offers a PRN schedule within an in-house program and opportunities to pick up additional hours as available, supporting residents’ mobility and independence.

Qualifications

• Current Ohio PT license in good standing.
• Graduate of an accredited Physical Therapy program.
• Experience in skilled nursing or long-term care preferred but not required.
• Excellent communication, documentation, and clinical decision-making skills.
• Experience with Net Health and PointClickCare (PCC) is preferred.

Responsabilités

• Perform comprehensive physical therapy evaluations
• Develop and implement individualized treatment plans
• Provide evidence-based therapy services to help residents improve mobility, strength, balance, and functional independence
• Collaborate with physicians, nursing staff, therapy team members, residents, and families
• Complete accurate and timely documentation using Net Health
• Utilize PointClickCare (PCC) as part of the interdisciplinary care process
• Maintain compliance with all federal, state, and company regulations
