Sunnyvale Post Acute is seeking a Physical Therapist Assistant (PTA) to join our rehab team. Full time, part time, and PRN opportunities available; new grads welcomed. Hourly compensation ranges from $40 to $55 per hour, with a supportive in-house therapy program.

Responsibilities include assisting the Physical Therapist, documenting progress, and collaborating with nursing staff to support patient discharge planning. We value teamwork, growth, and a positive work environment.

• Record daily treatment notes and weekly progress notes per PT Board.
• Communicate effectively with patients, families and health team members.
• Demonstrate compassionate, patient-centered care in all interactions.

Mansioni

• Record daily treatment notes and weekly progress notes per PT Board.
• Treat patients as directed by Physical Therapist.
• Assist in maintaining department.
• Participate in patient care and rehabilitation conferences.
• Assist with cleaning and maintenance of treatment area.
• Treat patients per the physician treatment plan.
• Instruct families or nursing staff in maintenance and caregiver training for discharge.
• Report problems with department equipment.
