Job summary

Storybrook Care & Rehabilitation in Fort Collins, Colorado is seeking a compassionate Physical Therapy Assistant to join our in-house therapy team. The PTA will implement treatment plans under the Licensed Physical Therapist, communicate patient progress, and contribute to care planning and daily documentation.

This in-house role supports a small, dedicated therapy team with low staffing ratios in a geriatric setting and offers a sign-on bonus for qualified candidates.

Responsibilities

• Treat patients according to the treatment plan of care developed by and under the supervision of the Licensed Physical Therapist.
• Communicate with the Physical Therapist any pertinent information concerning patient progress, concerns, and plans.
• Be proficient in therapeutic needs to treat patients based on their presentation and instructions given by the Physical Therapist
• Participate in patient Plan of care meetings if asked by the Director of Rehab.
• Record treatment notes, progress notes, provide objective input for discharge summaries and complete daily billing of services per policy and procedures.
